Undergraduate programme (FRESHERS):

Certified copy of GCE/SSCE/NECO o’level Result plus result online scratch card with Five – credits in related subjects to the programme anticipating for( including Mathematics and English Language) and not more than two-sittings.

Certified copy of Testimonial of secondary school Attended.

Certified copy of birth certificate.

Four (4) passport photograph with name and signature inscribe at the back.

Undergraduate Programme (DIRECT ENTRIES):

Direct entry candidate with NATIONAL DIPLOMA, NCE,HND etc are expected to submit:

Academic Transcript officially posted to the university Admission office/Regisrar.

Undergraduate Programme (Transfer Students):

All Transfer Students from other collaborated, Accredited, Recognize Institutions must present the following;

Official Transfer Letter from Head Of Department of previous Institution stating academic merit, character, reason for transfer etc and endorse officially with the institution seal.

Academic Transcript officially posted to the university Admission office/Regisrar.

(ii)Make request For the FORM by calling: +22967464335, +234-7065698255. We will required you to send sms your email address to the mobile numbers above requesting for FOPASE CODED ADMISSION FORM

(iii)Visit the FOPASE CAMPUS for the FORM:

N.B:

Fill the FORM and scan back via the emails with N5, 500 processing fee, Pin & Serial No. of your result online scratch card for confirmation of the result.
